2020-2021 Athletics Pre-Participation and Physical Information

Dear Parents/Guardians,

My name is Nate Roberts and I am the Assistant Athletic Director and Athletic Trainer at Pen Argyl. We are asking that you and your son/daughter login to our data entry system (sportsware online) to complete all of the pre participation requirements. You can follow the attached instructions for logging into sportsware for new athletes. If you have already established your account, you can login at using the username and password that you created. All information must be completed online or turned in by March 1st, 2021.  The first day of Spring sports practice is March 8th, 2021. All of the required forms can be found in your Sportsware account under the forms tab. The required forms have been made fill-able for your convenience. You may print and complete hard copies, or you may submit electronically through sportsware. Be sure to double check that you are submitting fully complete documents if you are completing online, once you submit you cannot make any changes. Also, please remember to complete or update your My Info tab in the sportsware account.

Signature Sheet:  This form states that you agree with all of the policies set in place by Pen Argyl Area School District and that you and your son/daughter will adhere to them. Please do not forget to sign the athletic accident insurance area at the bottom of the page. This is only to notify you that we do have athletic accident insurance available, should your son/daughter sustain an acute injury as a result of participation in their school related sport.

PIAA CIPPE:  This form is mandatory by the PIAA for participation in athletics. Sections 1-6, and 10 are mandatory for any athlete who has not yet participated in a school sport this year. Section 6 needs to be filled out by a physician and authorized after June 1, 2020. PIAA Section 7 is the ONLY required form for anyone who participated in a FALL or WINTER sport this school year, and can be found in the forms tab in a fillable version. If this form did not get uploaded to your account, and you played a FALL or WINTER sport please contact me. PIAA Section 8 is needed for anyone that sustained a significant injury/Illness requiring medical attention since the physical date and did not supply a clearance note to the Sports Medicine Staff. This form will be provided by Mr. Roberts upon request. PIAA Section 10 was added this winter and is a required form for all participants. It can also be found in the forms tab.

*Only PIAA forms can be accepted to satisfy the physical requirement*

**IMPORTANT**Anyone who meets the requirements for free or reduced lunch can find more information about obtaining a physical by contacting me via the email below.

**Baseline concussion testing, using the ImPACT software, is mandatory for any 7th, 9th, and 11th grade student, as well as anyone playing a school sport for the first time this season. Testing dates and times will be announced through school and our coaches at open workouts. Testing must be completed in order to be eligible to participate.

** A note from St. Luke’s University Health Network regarding obtaining a physical during COVID times: If you have been diagnosed with COVID-19 at any point in the past 90 days, please use your primary care physician to obtain your PIAA sports physical. If you have already participated in a fall or winter sport, and have been diagnosed with COVID-19 in the past 90 days please use PIAA section 8 or obtain a written clearance from your primary care physician regarding your COVID-19 diagnosis.**

Prior to participating on a team through the Pen Argyl Area School District, athletes must provide the Athletic Department with current address, emergency contact, insurance, medical alert and health history information. To expedite this process the Pen Argyl Athletic Department uses an online data entry system called Sportsware Online.

Go to

Scroll to the middle of the screen and click the Join SportsWare button.

Enter your School ID: 18072



Enter your First Name, Last Name, Email address and click the Send button.




Your request to join SportsWare will then be sent to the Athletic Trainer for review.


Once your request is accepted you will receive an e-mail with the Subject “SportsWare request accepted”.


Open the e-mail and click the link to continue to SportsWareOnLine.




Setting Your Password



Go to


Enter your Email Address and click the Reset Password button.


If you have multiple children in this system, you may use the same email to log in, but MUST create a different password for each child. *I recommend using the Student’s School ID # with two zeros in front. (example: 00123456)*


You will receive an

e-mail with the Subject “SportsWareOnLine Password Request”.


Open the e-mail and click on the link to reset your password. Enter your

e-mail address, new password and click the Save button.



Updating Your Information



Go to

Enter your Email Address and password, click the Login button.

Select My Info: Update your address, emergency contact and insurance information.



Start entering Athlete’s information. Complete the Athlete Online Access section by updating it with the Athlete’s email address (defaults with Parents initially). Then Update The Parent Online Access section with email and password. Continue to complete the remaining tabs.





Once done with My Info go to:


Med History: Complete a Medical History questionnaire.



complete required paperwork. Note: SportsWare will also display “You have ? forms to complete/download”.

Section 5 and 6 of the PIAA form must be printed and brought to your physical appointment.
